Please submit this form along with your payment by September 24, 2024.
We will seat children, students, & guests after that date but will not be able to guarantee that those people are sitting next to you.
Payments for extra seats, made payable to BIOZ, are due with the seat reservation form.
If you have a balance due on your account from last year and haven’t contacted the finance committee (financebioz@gmail.com) to set up a payment plan, we will not be able to issue holiday tickets to you. For any special financial arrangements, please call Sue Carre at 215-317-7751
