Please submit this form along with your payment by September 15, 2025.
We will seat children, students, & guests after that date but will not be able to guarantee that those people are sitting next to you.
Payments for extra seats, made payable to BIOZ, are due with the seat reservation form.
To make reservation off-line, please fill out Seat Reservation Form and mail it to BIOZ Seating Commettee, 1601 Borbeck Ave., Phila, PA 19111
If you have a balance due on your account from last year and haven’t contacted the finance committee (financebioz@gmail.com) to set up a payment plan, we will not be able to issue holiday tickets to you.
